SUMMARY:

Among all the techniques used by companies to prevent product launch failures and to identify risks on process developments, the Failure Mode and Effects Analysis (FMEA) method stands out regarding the study of quantifiable uncertain scenarios in the automotive sector, because is linked to some requirements of the IATF 16949 and became known as one of the automotive core tools. Even with a increasing number of papers and articles about FMEA in the last years, bringing improvements to the FMEA method and proving that is applicable in different sectors other than the automotive industry, hardly ever the researches discuss the specific topic about the process risk analysis after the FMEA is rolled out, which is a very important subject because the failures are evaluated by people and the manufacturing process naturally goes through changes as time goes by. Mainly after the unified FMEA VDA AIAG handbook release in 2019 until now it is there is no evidence of relevant work addressing this matter, so there is a lack of methodology capable of evaluating the potential process failure modes (PFMEA) and its documents in comparison to the production shopfloor reality after the manufacturing is running. In order to address this gap it was organized a research methodology to define a FMEA validation tool via reverse analysis of the documents and in loco process evaluation referred as reverse FMEA audit. After an initial literature review, two checklists were developed to approach machining and assembly process, and then a case study was performed at an automotive industry with the goal to demonstrate the proposed reverse FMEA technique step by step. So besides giving new insights about this quality core tool topic in discussion, the research also expects to reveal new failure modes or potential causes that weren’t mapped before at the PFMEA development. Up ahead in the conclusion, some future research recommendations would be the application of this FMEA reverse audit methodology in another manufacturing process, as well as using statistical analysis for confirmation and other means to compare the results.
