Reframing Artificial Intelligence in Healthcare: A Decolonial Design Approach to Reducing Digital Inequality in Low- and Middle-Income Countries

Artificial intelligence (AI) is transforming healthcare, yet its benefits are not shared equally. In low- and middle-income countries (LMICs), where the burden of disease is heaviest and innovation is most urgently needed, AI health tools usually arrive as ready-made products designed elsewhere. The main aim of this research is to develop a decolonial design theory by analysing documented AI healthcare implementations in low- and middle-income countries (LMICs), with the goal of generating a set of empirically grounded design principles to guide the development of fairer, locally relevant, and community-accountable AI health systems. This dissertation examines a question that research on digital health has largely overlooked: how do the design decisions embedded in AI health systems reproduce digital inequality, and how might design approaches informed by decolonial thinking reshape these systems toward fairness, local relevance, and community empowerment? The study builds its theoretical foundation on Arturo Escobar's pluriversal design and Sasha Costanza-Chock's design justice framework, and combines three complementary methods: an integrative literature review across four domains (AI in healthcare, digital inequality, data sovereignty, and decolonial design); a qualitative design analysis of documented AI health deployments in LMICs; and a design leverage mapping exercise that identifies the moments in the design process where different choices could redirect outcomes toward equity. The findings are expected to reveal three cross-cutting patterns and in response propose five design principles. This study contributes to design research by showing that digital justice, health sovereignty, and epistemic plurality belong inside the design process itself, embedded in requirements, constraints, and evaluation criteria, rather than being treated as political concerns external to it.
