ANALYSIS OF INTER-LIMB ASYMMETRY IN ANGULAR KINEMATICS OF THE LOWER LIMBS DURING GAIT IN INDIVIDUALS WITH PARKINSON'S DISEASE

SUMMARY:

Introduction: In individuals with Parkinson's disease (PD), gait asymmetry is an important and complex gait impairment. The asymmetry of spatiotemporal parameters during walking increases in individuals with PD compared to their neurologically healthy peers. However, most studies investigate gait asymmetry in spatiotemporal parameters. Although such a method can provide an easily interpretable summary of the entire gait performance, discrete values may not adequately reflect the complex changes in lower limb movement associated with pathological gait conditions. The study aims to analyze inter-limb asymmetry in the angular kinematics of the lower limbs during gait in individuals with PD. Methods: An open dataset was used for the gait of PD patients. 23 individuals with a clinical diagnosis of idiopathic PD participated in this study. Participants performed the tasks barefoot and wearing comfortable clothing. Participants walked on the ground on a 10-meter-long walkway at a comfortable, self-selected speed. Participants had not taken any medication for Parkinson's disease for at least 12 hours at the time of the experiment (medication OFF condition). Asymmetry was defined as the difference between the UPDRS scores in the OFF condition summed from the right and left sides of the body. The most affected side of the body was defined as the side with the highest UPDRS score. For the kinematic curves of the lower limb angles, we analyzed the temporal profile of the curve using statistical parametric mapping, comparing the most affected side to the least affected side. Results: There was a significant difference in step length, step time, and stride length, with lower values for the more affected side than the less affected one. There was a significant difference between the limbs in 31-46% and 71-90% of the gait cycle in hip flexion/extension, 26-47% and 67-78% of the gait cycle in knee flexion/extension, and 33-62 % of the gait cycle in ankle dorsi/plantar flexion.
