BIOMECHANICAL AND NEURAL ANALYSIS OF STEP INITIATION DURING COGNITIVE-MOTOR CONFLICT IN INDIVIDUALS WITH AND WITHOUT FREEZING OF GAIT IN PARKINSON'S DISEASE

RESUMO:

Parkinson's disease (PD) is a progressive neurological disorder resulting from the degeneration of dopaminergic neurons in the substantia nigra of the midbrain. Among the motor symptoms, the presence of a symptom called freezing of gait (FoG) stands out, characterized by brief episodes of inability to take steps or extremely short steps that normally occur at the beginning of gait. Some studies point to disturbances in the frontal cortical regions, the basal ganglia, and the midbrain locomotor region as the probable origins of FOG. However, exactly which neural substrates are involved in this symptom is not yet known. The lack of a deeper understanding of the brain regions involved in cognitive-motor processing during ecological activities such as taking a step makes interpretation difficult and, consequently, the development of more effective treatments. This study aims to understand better the cortical areas involved in the FOG through the biomechanical and neural analysis of step initiation during the motor-cognitive conflict. Thirty-seven individuals diagnosed with idiopathic Parkinson's disease participated in this study (16 with freezing of gait; 26 men and 11 women; mean age = 64.1 years, standard deviation = 9.2) and 17 healthy, age-matched subjects (6 men and 11 women; mean age = 63.9 years, standard deviation = 6.5). Hemodynamic responses of the supplementary motor area (SMA) and dorsolateral prefrontal cortex (DLPFC) were assessed using functional near-infrared spectroscopy (fNIRS) during a cognitively conflicted gait initiation task to select the foot for stepping (congruent conditions - CON and incongruent conditions - INC). The following variables will be analyzed: (a) time of onset of anticipatory postural adjustment (APA); (b) duration of the APA; (c) amplitude of the APA; (d) number of APAs; (e) number of errors in the task; and (f) first level analysis (GLM beta values) related to step initiation for SMA and DLPFC. For clinical scales, biomechanical and hemodynamic data, the analysis of homogeneity of variances and normality in the data distribution and residuals will be performed using the Levene and Shapiro-Wilk tests, respectively. In cases of non-normal data, the choice of data normalization method was selected from the Pearson P statistical function divided by the degrees of freedom; this ratio can be compared between the different forms of normalization and indicate in which of them the data follow the distribution closest to the normal (ratio close to 1). The two-way ANOVA analysis of the variance test will be used with the normalized data: 3 (group: Control, DP FoG+, and DP FoG-) and 2 (condition: Congruent and Incongruent). For the analysis of interactions and differences found in the ANOVA test, Bonferroni's post hoc will be used. Spearman correlations between hemodynamic, biomechanical, and Stroop responses (third part) will be evaluated by grouping the groups in each condition. The significance level for all analyzes will be set at 0.05. The analysis will be carried out with the R program.
